If you are interested in receiving coaching services from Advance LA, please complete the enrollment form after registration.
This form has been designed to provide us with information necessary to begin the initial assessment and enrollment process.

Advance LA coaching services begins with a comprehensive needs assessment that includes; the partnership meeting with the client, and family members, facilitated by Advance LA program leadership; followed by a one on one session between the client and an Advance LA team member. Once the initial assessment is complete the process of formalizing and prioritizing the client’s individualized goals is framed by the development of the coaching plan. Each client is then able to access support where they need it most through the implementation of innovative strategies and interventions provided by a member of the Advance LA coaching team.

Once your registration and application is complete, the credit card number that is provided will be automatically charged a one time $250 non refundable fee that covers the initial assessment process.

AUTHORIZATION AND AGREEMENT
I authorize investigation of all statements contained in this Application for Enrollment in ADVANCE LA. In the event of admission, I understand that false or misleading information, given in the application, or in any interviews, may result in rescission of any admission. I understand also that continued participation in ADVANCE LA services requires the applicant to abide by all rules and regulations of the program.
